Couple Therapy : An Information Guide

Are our problems serious enough to seek help?

For a number of reasons, couples tend to underestimate the severity of their problems. An intimate relationship is usually very important to a person and the thought that something might be seriously wrong with it can be quite threatening. The couple may mistake the problems for a nor-mal stage in the relationship.

Some people feel that the need to seek help is a personal failure and this view is unfortunately reinforced by the some-times negative social attitudes that linger towards people seeking this type of help. Finally, some people have difficulty or feel embarrassed about asking for help, question whether they deserve it, or whether their problems are “really serious enough.” If you think you might need help, it is worth investigating.

We are involved in a relationship that is just becoming serious. Is there anything we can do to prevent problems?

Some members of the clergy and couple therapists offer “pre-marital counselling” for couples who are beginning their relationship and want to take a preventive approach to problems. The better programs of this nature can help a couple anticipate and deal with some of the issues they will face before they become problems.

Is this just a phase we’re going through or do we really need help?

At one time or another all couples go through difficult changes and stages and experience stress. How much stress each person can, and is willing to, tolerate at any particular time varies. If you feel your stress level is intolerable or that you have reached an impasse, you should seek help.

Some people ignore problems and hope they will go away. Other people find they are unable to solve their difficulties despite repeated attempts to do so. If problems in your relationship persist, or if you can’t solve them in a way that is acceptable to both of you, do not wait to go for help. Problems are easier to resolve before they become larger and when there are still positive feelings in the relationship.

We’re intelligent people. Why can’t we sort out our own problems?

Feelings are in a different realm from logic and cannot always be resolved on an intellectual level. Sometimes an objective third party is needed to mediate conflict, or to help clarify what the issue means to each of the partners.

We’re fighting a lot and can’t agree on anything.
People who fight often feel as though they are going around in circles. Un-resolved issues that continue to bother a person can begin to colour everything, and to obscure the original causes of feelings of anger or outrage.

There may be aspects of the original problem that were fully or partly unconscious to begin with. These issues can become further obscured as both partners begin to resent each other’s anger, coldness, dissatisfaction, distance, etc.

The aim of couple therapy is to clarify the important issues in your relationship. Although you and your partner may not agree on all of them, therapy may enable you to identify the most significant problem areas, consider possible solutions and start implementing those that fit your situation.

We’ve been this way for years. What good will couple therapy do?

People develop patterns for handling difficulties. Some of these patterns fail to solve the problems; they increase tension instead. If both of you recognize a need for change and are prepared to do something about it, couple therapy can provide the extra support you may need during this difficult process. The therapist may also be able to offer a perspective that is not obvious to you and your partner.

Borderline Personality Disorder?

Borderline personality disorder is characterized by a pervasive pattern of instability of interpersonal relationships, self-image and affects along with marked impulsivity. Borderline personality disorder is the most common personality disorder and occurs in about 2% of the population.

It usually presents itself by early adulthood and is present in a variety of settings (e.g., not just at work or home). Symptoms usually flare up when a psychological stressor occurs, such as the threat of a break-up in a close relationship. Typically, when the stress subsides the symptoms subside as well.

Borderline personality disorder often overlaps with dysthymia (milder, longer lasting form of depression) and psychotic disorders. Nearly 3/4 of those suffering from the disorder attempt suicide or display self-mutilating behaviors like cutting themselves with razors or burning themselves. Only about 10% of suicide attempts are successful.

Symptoms of borderline personality disorder and risks of suicide are greatest during young adulthood and seem to diminish later in life. Most people with borderline personality disorder, especially those who receive therapy, achieve some stability in their life as they age.

Symptoms

The major symptoms or characteristics of borderline personality disorder are:

Unstable Personal Relationships

People with borderline personality disorder fear abandonment and make desperate attempts to maintain relationships. However, their perception of relationships is so unstable that a friend can become an enemy over an ordinary disagreement.

Unstable Self-Image

This disorder is characterized by marked uncertainty in major life issues, such as life goals, sexual orientation, values, career choices, or types of friends.

Unstable Emotions

Their emotional state can fluctuate dramatically from euphoria to intense anxiety to rage in a matter of hours or days. Typically, these emotional fluctuations are reactions to social interactions.

Lack of Impulse Control

Reckless impulsivity may cause people with this disorder to act in self-destructive ways such as driving dangerously or bingeing on food, alcohol, or sex.

Causes

Borderline personality disorder has as many causes as there are people who suffer from it. The disorder may be caused by a combination of a person’s parental upbringing, their personality and social development, as well as genetic and biological factors.

While the exact cause is not yet known, it is understood that the disorder most often manifests during increased times of stress and interpersonal difficulties in the person’s life. There is a common misconception that people with this disorder are weak or should be able to “snap out of it.”

It’s important to understand that those who suffer from borderline personality disorder are not consciously holding themselves back from correcting their behavior.

Treatment

Individual psychotherapy is the most effective treatment for this personality disorder. Treatment most often focuses on increasing the person’s coping mechanisms and their interpersonal skills. The more severe the disorder, the longer the duration of therapy.

Brief therapy can include a supportive, problem-solving approach, cognitive techniques to challenge distorted thinking, and interpersonal therapy to teach the person how to develop meaningful relationships. Brief therapy usually lasts four to twelve weeks.

Medication may be used to treat other accompanying disorders or specific symptoms the person may have, such as depression, psychotic symptoms, or anxious symptoms.

What Is Seasonal Affective Disorder?

Seasonal Affective Disorder (SAD) is a type of depression related to the change of seasons. It usually affects people during the fall and winter months, though some experience it during the summer months. Researchers disagree on the exact causes of SAD. One theory is related to an internal clock in the brain.

The pineal gland, a tiny structure at the base of the brain, secretes melatonin when there is a lack of light. The increase of light at dawn tells the gland to switch off this secretion so we wake up. SAD sufferers appear to have something out of balance in their internal clocks. They may also have a genetic vulnerability to the illness, or the illness may be triggered by stress.

Symptoms of SAD include change in appetite, increased craving for carbohydrates, weight gain, decrease in energy level, fatigue, oversleeping, irritability, difficulty concentrating and social withdrawal. Other symptoms of depression may also be present, such as feelings of guilt, loss of interest in activities once enjoyed, feelings of hopelessness or physical aches and pains.

Treatment of SAD often involves light therapy. In light therapy, individuals expose themselves to bright light, either through a specially designed light box or a light visor, for at least 30 minutes per day. Light from standard light bulbs or fluorescent lights is not sufficient.

Light boxes that produce the correct brightness needed are available from a variety of sources. Consult your health care provider to find a supplier who has a device that is appropriate for you.

Using tanning beds for light therapy is not recommended; tanning beds produce high levels of ultraviolet rays which can harm the skin and eyes. Side effects from light therapy may include eye strain, headache or inability to sleep, especially if the light is used late in the day. People with bipolar disorder or sensitive skin or eyes should use caution when using light therapy.

If you believe you may suffer from SAD, be sure to discuss any concerns with your health care provider. Do not try to diagnose yourself or begin light therapy without first talking to a health professional.

Even if you do not develop all of the symptoms of SAD, you may be affected by the short, cold days of winter. Getting plenty of rest, eating regular, healthy meals and having a consistent exercise routine will help you maintain good health during the winter months and throughout the year..
